Travel Vaccinations

If you require any vaccinations relating to foreign travel please fill out our Travel Health Assessment form. You can collect this from our reception or download it here. Once completed (and please complete one for each person travelling) hand it in to reception or email it to us at ibstockhouse@nhs.net.

Your form will be reviewed by our nurse. If you don’t need any vaccinations we’ll text you to let you know this. If you do need vaccinations then one of our Patient Services Team will ring you to book you an appointment with our nurse.

It is important to fill in your form as early as possible  – it must be returned at least 8 weeks before you travel.  This is so that we have time to process your form, book you in  and for any vaccines you need to have time to work.

Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge.  This is because not all travel vaccinations are included in the services provided by the NHS. The nurse will let you know if this is the case at your appointment.
