New Patients

If you live within our practice area we are happy to accept you as a patient.

There is quite a bit of paperwork to fill in to become registered and so please pop in to collect a pack for each member of the household who wishes to register or download from the links below. Once you’ve completed the paperwork send it back to us (drop it in to reception, put it in our letterbox or scan and email) and we’ll get you registered. It’s very straightforward)!

Anybody on regular medication will need to have a telephone consultation with one of our practice pharmacists before they can be prescribed more.

Adult Registration pack:

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register you if you haven’t completed all relevant parts.

1. Download and complete the Adult New Patient Registration Form

2. Download and complete the NHS GMS 1 form

Once both forms have been completed for each patient, they can be emailed to: ibstockhouse@nhs.net

Child Registration pack (Children 0-16 years)

Please fill out all parts of the registration pack once you have downloaded it. We may not be able to register your child if you haven’t completed all relevant parts.

1. Download and complete the Child (0-16) New Patient Registration Form

2. Download and complete the NHS GMS 1 form

Once both forms have been completed for each patient, they can be emailed to: ibstockhouse@nhs.net
