Long term conditions, also known as chronic care, are health issues that last a year or more and require ongoing medical attention. Long term conditions may include:
Cardiovascular disease: Such as hypertension, heart disease and stroke
Diabetes: A condition that affects how the body processes blood sugar
Chronic respiratory diseases: Including chronic obstructive pulmonary disease (COPD) and Asthma
Arthritis and musculoskeletal disorders: Conditions affecting the joints and muscles, like rheumatoid arthritis
Mental health disorders: Such as depression and schizophrenia
Chronic kidney disease: A progressive loss of kidney function over time
Management of these conditions often require regular medication, life-style changes: diet, exercise and smoking cessation are critical components of managing many chronic conditions. Regular monitoring, with frequent check ups and essential to mange these conditions effectively.
Each year around your birth month, you will usually be recalled to have your annual appointment that is specifically designed for your long term condition.
